I had the same problem with my GH.
After searching for a long time, I placed a ladder inside where the grating is located.
At the very back, towards the poop deck, there wasn't even a ladder.
I wouldn't put the ladder in the middle. If someone asks where your ladder is, just say it's under the grating.
There are photos of my model, but I have to look for them on the PC. The build log was lost on msw.

This might be a good solution
The hatches I left intact could indeed have ladders under them and they would allow access up and down to forecastle and poop. If I pretend there is a ladder under the grating on the quarter deck, then we have easy access everywhere (and out of the weather!)
